4-H Record Book Forms
Here you will find the county forms to complete your record book. The record book forms are available in two formats: in word and in a pdf. The word document can be completed on line, saved to your computer and printed. The pdf must be printed off to be completed by hand or on a typewriter. Feel free to download as many as you need to complete your record book.

Club and Committee Financial Forms
Each year 4-H Clubs and Committee are required to submit two financial forms to the UWEX Office: the Annual Financial Report and the Financial Record and Inventory

Use this form to document the awards that you would like to give out to exhibitors in your club for the various projects. Please turn into the Extension office when completed.
This form is used county-wide for Record Book and Member evaluation. If you have questions on how to use this form feel free to give Polly in the Extension office a call.
